Serving as a vital link for members transitioning from behavioral health care, the full-time remote Transitions of Care Behavioral Health Care Coordinator will manage appointments, connect members with community providers, and ensure continuity of care during the critical post-discharge period. Key responsibilities Acting as the "care traffic controller" to track clinical needs and schedule appointments for members Conducting telephonic outreach to support members and provider offices with genuine care Building relationships with community behavioral health partners to confirm service availability and engage on members' behalf Required qualifications Associate's degree and 3+ years of experience in a healthcare setting Ability to manage a fully remote schedule, M-F, 8:30am-5:00pm CST, MST, or PST Comfort with multitasking on the phone, including listening, talking, and typing simultaneously Familiarity with electronic health records and Google Workspace tools A collaborative mindset with the confidence to advocate for members effectively
