Hospice Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN) – Per Diem

Provide comfort, dignity, and compassionate care when it matters most.
We’re seeking caring and dependable Hospice LPNs/LVNs to support patients and families during end‐of‐life care. In this role, you’ll work under the supervision of a Registered Nurse and alongside an interdisciplinary hospice team to deliver comfort‐focused, patient‐centered care.

Schedule

• Per Diem
• Flexible shift options available
• 4‐hour, 8‐hour, or 12‐hour shifts
• Day, evening, or night shifts based on assignment needs

What You’ll Do

• Provide direct hospice nursing care under the supervision of a Registered Nurse
• Implement individualized plans of care focused on comfort, dignity, and quality of life
• Administer medications and treatments per physician and RN orders
• Assist with pain and symptom management and monitor patient response
• Perform routine patient assessments and promptly report changes in condition
• Provide emotional support and reassurance to patients and families
• Reinforce patient and family education related to care plans and symptom management
• Collaborate with physicians, social workers, chaplains, and interdisciplinary hospice team members
• Document care activities, observations, and patient responses accurately in the medical record

What We’re Looking For

• Active LPN or LVN license in the state of practice
• Minimum of 1 year of recent hospice, palliative care, or acute care experience required
• BLS or CPR certification required
• Strong clinical observation and documentation skills
• Compassionate, patient‐centered approach to end‐of‐life care
• Ability to work independently while collaborating with interdisciplinary teams
• Travel may be required for home hospice assignments
• Inpatient hospice roles are facility‐based and do not require travel
